"Asia has long been a leading destination when it comes to medical tourism, and people from all around the world visit the region every year, lured by lower cost procedures and high medical standards. With strong governmental support for their medical tourism industries, Malaysia and Singapore have grown considerably in significance over the past decade or so. And while Thailand remains one of the most popular countries to visit, in recent years several unexpected players have started to emerge as viable alternatives. Today, markets including India, Japan, Korea and China are all strong contenders, but are they right for you? Melinda Earsdon spoke to leading surgeons in each of these markets, asking for their thoughts on the local landscape, the pros and cons of their country as a medical tourism destination, and how to avoid a bad experience...
KOREA


Q. How would you describe Korea’s cosmetic surgery landscape?


In Korea, if you want to get into medical school you need to rank among the top one percent of students, and only one percent of these students go on to become plastic surgeons. As a result, the market is fiercely competitive and so we have to continuously study and develop new surgical methods in order to survive. Because of this we have some of the finest surgeons in the world.

There are more than 1,000 plastic surgeons here and the number of dedicated hospitals is increasing every year. Thanks to the “Korean wave”, we have also seen an increase in medical tourism and a growing number of visitors come to Korea for plastic surgery and aesthetic reasons. As a result, many plastic surgeons now work hard to grow their business overseas.

Q. Is cosmetic surgery increasing in popularity in Korea?


In Korea, plastic surgery is seen in a more positive light these days. In the past, people tended to hide the fact they’d had work done, but nowadays they no longer try to conceal it, even celebrities. The general public has started to become more aware of how plastic surgery can help restore confidence, which means the number of patients and the surgeries performed are on the rise.

Q.What are the most popular treatments and procedures patients request?


The latest trend in Korea is two-jaw surgery. Many people with a square, overdeveloped jaw want to make their face look smaller or have a slimmer chin, and I can do this by trimming the chin line. Two-jaw surgery is also popular with people who have a protruding chin and mouth, facial asymmetry, and jawbone problems, because it can reshape both the upper and lower jaw simultaneously.

However, eyelid and nose surgery are also in high demand, primarily because Asian people have smaller eyes and a flat nose, compared with Caucasians.

Q. Has Korea produced any groundbreaking new procedures?


Several procedures have been developed to make the eyes bigger, and techniques such as the non-incisional method, inner corner extension, outer corner extension and outer corner reshaping can be applied on an individual basis.

The non-incisional method can create a double eyelid literally without making any cuts, so it doesn’t leave any scars. Small eyes can be made larger by removing the Mongolian fold that covers the inner corner, while outer corner extensions can stretch the width of the eyes. Outer corner reshaping can also make the eyes look bigger by lowering the outer surface of the eyelids.

Q. What new trends do you predict we will see in Korea in the next year or so?


In Korea, people are really interested in anti-ageing, and I predict non-surgical procedures such as botox and fillers will be in high demand. Rejuvenation surgery such as wrinkle removal and facial contouring will be also popular.

Q. How would you rate Korea as a medical tourism destination?


I believe Korea plays home to some of the most talented cosmetic surgeons in the world, and our skills have been developed specifically with Asian faces and body shapes in mind. As a result, it challenges even established markets such as Thailand and Singapore.

Q. What are the pros and cons of choosing Korea for your surgery?


Given the continuing surgical developments in Korea, we offer outstanding techniques and procedures at a lower cost than many other developed countries. However, visitors should make sure they spend extra time here just in case revisions are required after surgery.

Q. What advice would you give to patients looking to visit Korea for surgery?


There are a few things to check. The hospital must be qualified and have board-certified surgeons on call, and there must be open channels of communication with your surgeon. But mostly, my advice is to
make sure you are undergoing surgery for yourself, not for somebody else or to fit into a mold.

There are already many people who have had and are interested in Double Eyelid Surgery.
Many people can see a dramatic change just from this one procedure, meaning it is popular among both men and women.


But there are also many people interested in having Revisional Eyelid Surgery. This is usually when the shape and/or height of the eyes are different, or from any other problems that may have occurred following surgery.

▶ Cases Requiring Revisional Eyelid Surgery

Thick Double Eyelid Fold

Double eyelids may look thicker due to swelling after surgery. However, if the eyelids continue to look swollen after 1 year then Revisional Eyelid Surgery should be considered.

Uncorrected Ptosis
The double eyelid crease, including the scar from the incision can be lowered or the excess skin between the double creases can be removed.
If it has been less than 2 weeks since surgery, then the condition can be improved by removing the tied knots. If it has been over 2 weeks, then the surgical site has already healed meaning that an incision needs to be made for revisional surgery.

Through an incision, ptosis can be corrected along with lowering the height of the double eyelid crease using the appropriate method to remove an appropriate amount of skin.

Asymmetrical Double Eyelid Fold

Asymmetrical eyes are very normal, but if the asymmetry becomes more severe over time then Revisional Double Eyelid Surgery should be considered.


Asymmetrical Double Eyelid Fold
Loosening of One Side
Difference in Levator Muscles due to Ptosis on One Side
New double eyelid is made on the desired site after untying the existing double eyelid crease.
The crease can be adjusted to balance the other side.
Ptosis correction is performed to balance the levator muscle in both eyelids to create more symmetrical double eyelid creases.

Indistinct or Reversed Double Eyelid Crease

Despite thick skin or excess fat on the eyelids, if double eyelid surgery using the non-incisional method is performed without removing excess fat, the crease can become indistinct or reversed.


Requiring Fat/Muscle Removal
Not Requiring Fat/Muscle Removal
The tissue between the previous and the new double eyelid crease can be removed after lowering the crease through an incision.
2-3 cm of partial incision may be required.

Multiple Double Eyelid Creases

Multiple creases on the eyelids may appear unpleasant so a corrective procedure should be considered.


Surgical Method
After removing all existing creases, a single crease is created by untying the adhesive tissue.

* Low Crease or Thin Fold

Due to the aging process, droopiness of the skin may cause double eyelids to become lower or thinner from the time of surgery. Also requesting a thin fold at the time of surgery due to anxiety may result in dis-satisfactory results after a long period of time.

Thin Fold Due to Skin Above Incision Line Covering the Crease
Condition can be fixed by untying the existing crease or making another crease over the existing one.
Condition can be fixed through an incision to remove any droopy skin.

No Change, or Appearance of Smaller Eyes After Surgery

When double eyelid surgery is performed without an awareness of ptosis or short length between the eyes may cause the shape of the eyes to appear very round and dis-satisfactory.

Condition can be corrected to strengthen the levator muscle and connective tissue by creating a new double eyelid crease which must be smaller than the existing crease.
Horizontal and vertical length of the eyes can be balanced by lengthening the width through an inner corner fold removal, outer corner extension, or an outer corner reshaping.

Severe Scarring After Double Eyelid Surgery

Scar induced from double eyelid surgery usually disappears within 3-6 months. However, causes for severe scarring can be due to inappropriate suture methods or the condition of the patients skin (keloid).


Surgical Method
Except for keloid, the condition can be improved by an incision to remove any scarring, in the case that the scare was induced from inappropriate suture methods, it is followed by delicate suturing.

 
Revisional Double Eyelid Surgery differs for each person depending on one's condition, and the time that has passed from their first previous procedure.

It's also important to decide on the right procedure for you after a thorough consultation with a specialist.

You should at least wait 3-6 months from your previous procedure before thinking about Revisional Double Eyelid Surgery.

But in the case that the results of the surgery are dis-satisfactory immediately after surgery then it is recommended that a Revisional Eyelid Surgery be performed within the first 2 weeks following surgery.
