Cause of Revisional Nose Surgery and Treatments

* Contracture on Nose Tip

Symptoms: In cases of infection, if the infection is neglected a scar will form on the inner structure causing the nose to harden.

The nose may then become short and stiff if not treated properly.

Treatment: After infection causing implants are replaced with autologus tissues (corium, fat or rib cartilage).

For nose tips turned upwards, the tip is reshaped using autologus septal cartilage or rib cartilage.

* Imbalance of Nose and Face

Symptoms: The nose does not go well with the face and the nose was made too high, or the nose tip too sharp.

In the case that the nose is to big compared to the face, or the surgery did not goes as planned.

Most patients are unsatisfied with such results in which case revisional surgery should be considered.

Treatment : Special care, and precise planning and techniques are required to carry out size reduction or relocation of the implants.

* Deviated Implant

Symptoms: The nose appears bent or unnatural due to the movement of the implant from the nose bridge.

In the case that the implant moves upward, the nose may appear crooked and unnatural.

Treatment: A new and proper implant is put in place of the older one.

The site of the misplaced implant must also be treated.

* Allergic Reaction to Silicone

Symptoms: Those who are unaware that they are allergic to silicone, yet proceed with the procedure, may experience an allergic reaction including prolonged swelling, and fluid retention around the nose bridge.

Treatment: Although allergic reactions are not easily identified, those with an allergic reaction will have their silicone implants immediately removed and replaced with autologous tissues (corium, fat, or rib cartilage).

* Nose Tip Deformation Caused by L-Shaped Silicone

Symptoms: When the L-shaped silicone implant is placed on the nose tip, the pressure on the cartilage can cause deformation.

The skin on the tip may become reddish and the implant may become visible or even protrude.

Treatment: The L-shaped silicone is replaced with another implant that will only be inserted in to the nose bridge area.

Autologous cartilage is used for the tip of the nose.

The skin affected by the deformation of the nose may be further damaged unless it is reinforced by autologous cartilage or alloplastic dermis.

Significance of Revisional Nose Surgery

* Autologous Cartilage

Artificial implants for the purpose of revisional surgery can result in operative failures, infection, or deformations.  Hence, the use of autologous cartilage is recommended.

* Tissue Preservation

It is important to minimize the separation of tissues during revisional surgery in order to preserve them.  This also reduces the recovery time needed after surgery, and also helps to maintain a natural nose shape by minimizing tissue damage.

* Understanding the Cause
Even at the hands of experienced surgeons revisional surgery is a very delicate and difficult operation.  It is important to be able to understand the cause for failure of the previous surgery in order to have a safe and successful procedure.

Time Period for Revisional Nose Surgery

Revisional surgery is generally recommended 6 months after the preceding surgery when tissues become tender.

Revisions of relatively simple surgeries, such as nose bridge augmentation, can be performed within 3 months of surgery.  In the case of infection, revisions should be postponed until the implant is removed and inflammation has gone down.  Revisional surgery must be approached with caution since it is likely to damage the tissues more than the first surgery.  It is also important to note that an excessive adjustment can result in another revision.

Because Revisional Nose Surgery can cause more damage to the tissues than the first surgery, it is important to pay close attention to the time that has passed from the first surgery.
